Contracting
You can make the contracting request above or by emailing [email protected]. Your request to get contracted will trigger an email with a link. Simply click on the link in the email to begin the contracting process.
For the Medicare Supplement and Health products, the pre-appointment states are AL, LA, OH, PA and MT. This means that you need to be appointed in these states prior to being able to take any client application. Any other state appointment will be activated when new business is received.
For Annuity sales, agents are required to have a Life license, Errors and Omissions insurance, Anti-Money Laundering training, NAIC Suitability and Best Interest and will need to watch a quick product training video. These need to be in place or taken care of prior to taking any client application.

Transfer/Release Policy
If an agent wants to move their contract to be under our hierarchy, it is best to get a signed Release Letter from their current upline.

- Heartland will process an immediate transfer if they have NOT written business in the prior 90 days.
- If they have written business in the prior 90 days, they can fill out a 90-day intent to Transfer Form. Once 90 days is up, Heartland will recontract the agent under the new upline.

Add Non-Resident State

EFT Changes
You will need to email Heartland Licensing directly to ask that a change be made to your EFT information. They will likely provide you with an EFT form to fill out and return to them.

Assignment of Commissions
Complete the Humana Assignment of Commission form and send to [email protected] and [email protected]. Please include “Commission Assignment Form” and your SAN # in the subject line of your email.
